  1 JAN 1981 - 31 DIS 2000  

Sambungan dari  TAJUK AF 037.

Di bawah Tajuk AF 037, koleksi kes kemalangan udara yang melibatkan pesawat TUDM adalah meliputi kes yang berlaku mulai tarikh TUDM ditubuhkan sehingga 31 Dis 1980.  Manakala Tajuk AF 038 ini kes-kes kemalangan yang berlaku mulai 1 Jan 1981 hingga kini.

Saya juga maklum bahawa mungkin catatan saya ini akan menyentuh perasaan mana-mana ahli keluarga kepada individu yang terlibat dalam mana-mana kemalangan udara yang dipaparkan.  Sekiranya berlaku demikian, sila emelkan kepada saya supaya nama-nama individu tersebut akan saya padamkan dari catatan saya.

Saya berpandangan positif kerana dengan membaca atau terbaca nama-nama mereka di bawah, akan boleh mengingatkan kita (rakan-rakan, saudara mara atau anak cucu) untuk mendoakan mereka agar ruh mereka (yang beragama islam) akan dicucuri rahmat oleh Allah azzawajalla. Kalau kita teringat kesilapan mereka, kita akan jadikan sempadan, usah diperkatakan lagi. Hapuskan rasa dendam peribadi. Kita kenanglah jasa mereka, kebaikan mereka, perbualkanlah kebaikan mereka. Maafkanlah segala kesalahan mereka dan doakan kesejahteraan roh mereka.

Data/maklumat di bawah saya perolehi dari pelbagai sumber, akhbar, majalah, website, arkib akhbar-akhbar lama dan sebagainya. Terima kasih kepada rakan-rakan yang membantu memeberikan sumbangan berita dan juga memperbetulkan fakta yang salah. Masih banyak yang tidak lengkap. Saya juga mengharapkan pembaca yang mempunyai maklumat yang lebih tepat untuk membantu saya.

Dalam usaha mengumpulkan maklumat berkaitan kemalangan pesawat TUDM saya mendapat sokongan dan bantuan dari seorang rakan facebook yang memmpunyai minat yang tinggi mengumpulkan keratan akhbar berkaitan TUDM sejak peringkat awal TUDM (TUDP) ditubuhkan.  Beliau ialah Kapt Dr Ahmad Damanhuri bin Ibrahim, Pegawai Pelajaran Daerah Kinta Selatan, Perak.  Setinggi-tinggi penghargaan dan terima kasih di atas sumbangan yang amat bernilai dari beliau.

Bagaimanapun beberpa maklumat yang diperolehi dari internet/website didapati kurang tepat atau salah dari segi tarikh, tempat dan nama-nama pilot yang terlibat dalam kemalangan yang disebutkan, terutamanya kemalangan yang melibatkan pesawat PC7. Aku sedang berusaha untuk mendapatkan maklumat yang lebih tepat dan memperbetulkan kesilapan.  Terima kasih kepada pembaca yang budiman yang meluangkan masa membuat pemerhatian dan teguran di atas beberapa kesilapan yang berlaku.

Sejarah adalah sejarah.  Ada sejarah bersifat negatif ada yang positif.  Ia akan menjadi pengajaran kepada generasi mendatang. Semoga tidak akan ada insan-insan yang menggunakan maklumat dalam blog ini untuk membuat cerita negatif, kepentingan diri, bahan gimik berpolitik, memperlekehkan organisasi TUDM atau sebagainya.  Usah berseronok dengan penderitaan orang lain..

1 JAN - 31 DIS 1981

30 Jan 1981

One Bulldog on training flight was crashed on the school field in Perlis and the trainee pilot, Lt M (U) Mohd Effendi bin Sulong TUDM killed. Allahyarham Affendi was my senior in Officer Cadet School, Port Dickson (TJP 36) and also my senior in No 1 FTC Alor Setar.  The crash incident happened couple of days after I left 1 FTC for my helicopter flying training in No 2 FTC Kluang.

This crash happened about a week after I left Pulatibang I TUDM Alor Setar for Pulatibang II, TUDM Kluang. He was my intermediate senior during my short tenure in Pulatibang I.

10 Feb 1981

One Tebuan jet from No 9 Sqn based in Kuantan was crashed during training flight and killed its pilot, Lt (U) Mohd Razif bin Abdulah.  This crash happened 10 days after the incident that killed LtM (U) Mohd Affendi in Perlis.

25 Apr 1981

S61A(FM 17-12) crashed at Gunung Silabu hillside, near Kuching Airport -2 killed 13 survived (the casualties were the co-pilot, Lt (U) Zakaria Shaaban and the AQM, Sjn Kanagaratnam a/l Supramaniam.  Note:  Couple of months after this incident (20 Oct 1981) I reported to RMAF Kuching to take up the appointment of DAMO.

11 Jul 1981

S61A Nuri (carying out the task of OPS Ukur Sempadan) crashed at Batu Melintang, Perak. All 4 crews survived. One of the 3 passengers from the National Department of Survey and Mapping killed in this crashed.

1 JAN - 31 DIS 1982

6 Jun 1982

One RMAF F5E jet (29-06) was forced to land at Butterworth air base runaway due to technical  difficulties, the pilot, Mej (U) Victor Emmanuel survived and aircraft severe damaged and written-off.

24 Sep 1982

On September 24, 1982, Caribou aircraft M21-17 (formerly FM 1415), callsign Misty 116B took off at 1450 hours from Taiping airstrip for an air drop supply mission. On board were the Pilot, Co-pilot, one Air Quarter Master and 4 Air Dispatchers.

The aircraft took off at 28,388 lbs all-up-weight and with 4 hours endurance. The aircraft was enroute for Atap, Gedung and Tenau cruising at 3,500 ft. At 1505 hrs, the aircraft called on the RT, reporting east of Ipoh, then switched over to RMAF operational frequency, informing Taiping that they were 5 minutes out of Brinchang and established at ops area.

That was to be the last call heard from Misty 116B. Contradicting emergency procedures, DISTRESFA was only activated at 1900 hrs and Search and Rescure was initiated. FM 1415 was sighted, crashed 6 1/2 miles North-east of Atap on September 25, 1982 at 1040 hrs. There were no survivors from the accident (Kapt Kalam Pie).

All the passengers were from Air Despatch Unit in Taiping. (*Both pilots were Kuchingites. I was serving in Kuching at this time. So, all the RMAF Kuching officers were involved in the funeral ceremony (fall-bearers).

4 Sep 1984

One RMAF PC7 aircraft in training flight crashed near Pendang, Kedah and  the student pilot safe.

5 Okt 1984

One MB339A aircraft, RMAF Kuantan Base, Malaysia

Aircraft crashed just after take-off. Encountered Wake Turbulence. Lt (U) Othman Mat Taib and Lt M (U) Rashid Majid ejected and both ejected safely at 50 feet AGL.


1 JAN - 31 DIS 1985

14 July 1985

MB339A trainer jet from Pulatibang 3 crashed into the village houses near Kuantan Base during the aerobatic show in conjunction with the 27th Anniversary of the Air Force - both the pilot Mej (U) Shubli bin Ismail of Kedah and his assistant, Kapt U) Kamaruzzaman  bin Mohd Noor of Melaka were killed. Sadly, I was working as Logistic Officer in Pulatibang 3 at that time. I knew both of them very well.

24 September 1985

One A-4PTM Skyhawk crashed in RMAF Kuantan.  The aircraft developed engine problems on landing and the aircraft skidded off  runway while landing at air base. Wings torn away and aircraft exploded. The pilot Lt (U) Khairil Asri bin Baharin ejected at low level and safely landed on the runaway. Khairil is my intake.

3 February 1986

Cessna 402B from No 2 Sqn RMAF Subang crashed in bad weather in eastern Sabah. The aircraft hit the side of Bukit Ibol in poor weather at Sabah-Sarawak border and all eight on board killed.

21 October 1986

One RMAF 2-seater F5F jet (29-15) crashed near Kertih, Terengganu during dissimilar air combat tactic (DACT) training sortie.  The pilot Kapt Tengku Adli Shah ejected safely but his co-pilot Lt Azman bin Jaafar TUDM failed to eject and killed. The flight was part of Elang-Kiwi Exercise. The late Azman (nickname: Rock) was together with me when I was working as Logistic Officer of No 3 FTC.

RMAF fighter pilot, Lt Affendi bin Buang TUDM ejected safely from the A4-PTM Skyhawk he was flying, over Kampung Budu, Benta near Kuala Lipis, Pahang during a bombing exercise. Affendi is now holding the rank of General and he was appointed as a Chief of Air Force in 20 Dec 2016.

23 June 1988

One of five Skyhawk A4PTM from No 6 Sqn, Kuantan AFB in formation flight from Kuantan AFB to Labuan AFB dissappeared after they entered some weather about 290 nautical miles from the base in the South China Sea about 290NM from Kuantan. The aircraft wreckage and the body of the pilot, Lt Wahi Anuar bin Mat Amin TUDM was not found (UNTIL TODAY). The late Wahi was together with me when I was working as Logistic Officer of No 3 FTC.

Selepas tujuh tahun, iaitu pada 8 April 1996, Mahkamah Tinggi Kuala Lumpur membuat pengisytiharan bahawa Lt Wahi telah dianggap telah meninggal dunia.

September 1988

One A4PTM aircraft crashed (need further information) - may be this one involved Lt Felix Gonsalvez TUDM (now Kol).  He ejected safely. Felix is my intake.

3 October 1988

One RMAF A-4 Skyhawk from RMAF Kuantan air base crashed. Three aircraft flight from Kuantan to Labuan for operational detachment was crashed into the South China Sea in bad weather.  The pilot,  Kapt Abdul Mues Abdul Samad TUDM  ejected and picked up after 15 hours in the sea by RMN ship KD LEKIR near the town of Miri, Sarawak

14 November 1989

One RMAF Nuri S61A crashed between Gunung Gerah and Gunung Bilah in Perak- all 21 (air crews and passengers) were killed including the pilot, Kapt James anak Tingom TUDM, Kapt Shuhaini Ahmad TUDM (co-pilot) and Sjn (U) Zainuddin Ahmad Samad (AQM).  The late James Tingom was my super-senior in No 2 FTC, RMAF Kluang in 1981.

18 December 1989

One S61A Nuri helicopter under military communication flight crashed at Sg Lundu, Sarawak - all 16 crews and passengers were killed - (among the victims were:

(2)   Brig Gen Dato Hasbullah Mohd Yusuf (3rd Brigade Commander),

HARI Isnin, 18 Disember 1989. Cuaca sebagaimana kebiasaan pada musim tengkujuh di Kuching, Sarawak masih tidak menentu dan beberapa kawasan rendah sudah ditenggelami air berikutan hujan lebat beberapa hari sebelumnya.

Di Pangkalan Tentera Udara Malaysia (TUDM) Kuching pula, sebuah helikopter Nuri seawal pukul 8 pagi sudah bersedia menerbangkan 16 penumpang termasuk empat pegawai tinggi Angkatan Tentera Malaysia (ATM), masing-masing dua orang berpangkat Jeneral dan tiga Kolonel.

Mereka terdiri daripada Panglima 1 Divisyen, Mejar Jeneral Datuk Mustaffa Awang; Panglima 3 Briged Infantri, Brigadier Jeneral Datuk Hasbullah Yusuf yang lebih dikenali sebagai Bond Hasbullah; Pegawai Memerintah Batalion Kelima Rejimen Renjer Diraja (RRD), Leftenan Kolonel Mohd. Johari Selan; Pegawai Memerintah Batalion Ke-13 Rejimen Askar Melayu Diraja (RAMD), Leftenan Kolonel Tunku Abdul Aziz Tunku Osman dan Leftenan Kolonel Mustafa Kamal Mohd. Yusoff.

Destinasi yang dituju mereka ialah Kem Sematan untuk menghadiri majlis penutup Kursus Risik Tempur. Namun, berikutan halangan cuaca buruk, penerbangan helikopter pelbagai guna itu terpaksa ditangguhkan sehingga pukul 8.50 pagi.

Penangguhan hampir sejam itu rupa-rupanya memberi petunjuk bahawa penerbangan berkenaan adalah yang terakhir buat kesemua penumpang.

Berdasarkan rekod kenyataan rasmi Kementerian Pertahanan, helikopter itu terputus hubungan dengan menara kawalan hanya selepas 15 minit berlepas sehingga TUDM terpaksa melancarkan gerakan mencari dan menyelamat (SAR).

Dua buah pesawat Alouette telah dikerah untuk mengesan Nuri yang membawa individu penting dalam hierarki kepimpinan ATM itu sehinggalah pencariannya menemui noktah. Ia ditemui terhempas di muara Sungai Lundu, berhampiran Kampung Kuala Bandang, kira-kira 60 kilometer (km) ke barat laut Kuching.

Kejadian yang mengorbankan semua penumpang itu dilaporkan berlaku dalam keadaan cuaca buruk dengan hujan lebat disertai angin kencang dan kilat sabung-menyabung

15 August 1990

One A4 Skyhawk jet from No 9 Sqn, Kuantan Air Base which was on ground attack tactical training crashed into the mountain range around Gunung Dulang 77 km northwest of Pahang and killed its pilot,  Mej Ong Cher Kiong TUDM.

Sebuah pesawat C130H (30-03) yang membuat tugasan kumpulan penerbangan komunikasi VVIP (rombongan lawatan Agong ke Sarawak) yang diketuai oleh Kapt Ahmad Razlah TUDM telah tergelincir dan terbabas di lapangan terbang Sibu. Seorang penumpang awam (seorang jururawat) terbunuh.

20 September 1990

Sebuah pesawat helikopter jenis Agusta A109C mendarat cemas di Lapangan Terbang Subang kerana masalah tail rotor yang tidak berfungsi. Juruterbangnya, yang juga seorang Test Pilot, Lt Kol Lim Tow Liang TUDM mengalami luka-luka ringan dan pembantu juruterbangnya, Kapt Sidek bin Abas TUDM mengalami kecederaan tulang belakang.

25 September 1990

One RMAF PC7 training aircraft piloted by Lt Muda Thulasiram a/l Subramaniam TUDM crashed in the padi field at Kampung Bukit Rambai, Pendang, Kedah.  Thulasiram, a student pilot killed in the crash. 


1 JAN - 31 DIS 1991

1.  16 January 1991 - Nuri S61A did an emergency landing in the rubber estate at Parit Mat Zin, Muar, and all the 12 passengers safe unhurt.

2.  10 June 1991 - S61A Nuri crashed at Banding, Perak - all 6 crews and passengers perished. The aircraft captain, Kapt Tan Eng Huat TUDM, co-pilot Lt Saiful Bahri Baharum TLDM, AQM Flt Sjn G. Balasubramaniam were killed. Bala died  22 days after the crashed due to serious burned.The other 3 passengers were Kapt Sarwant Singh, LKpl Mokhtar Mohd Zain and Pbt Johar bin Mohd Zain from 304 Bn (TA) also killed

3.   11 July 1991. Sebuah pesawat Nuri S61A yang membawa  7 krew dan penumpang (3 penumpang awam dari Jabatan Ukur dan Pemetaan Negara) yang membawa bahan binaan untuk batu sempadan (Ops  Ukur Sempadan) telah terhempas di Batu Melintang, Kelantan. Seorang penumpang, kakitangan Jabatan Ukur telah terbunuh. 

4.   22 Oktober 1991 - A routine flight in preparation for LIMA 91 exhibition ended in tragedy when RMAF pilot, Kapt Suhaimi bin Mat Sahid TUDM was killed when his Pilatus PC7 aircraft crashed in Kampung Titi Besi, Kedah.


1 JAN - 31 DIS 1992

1.   29 January 1992 - a PC7 aircraft on the light attack exercise crashed near Cukai, Terengganu. The pilot, Lt M Hamir Fizar bin Yusof TUDM killed.

2.    31 January 1992 - One PC7 training aircraft crash landed near a house in Kampung Bukit Tinggi, in Kepala Batas.  The instructor pilot, Kapten Ahmad Redzuan Mohamad Marzuki TUDM was able to release himself from the seat and egress the aircraft and the trainee Lt Muda Abang Mohamed Ali Badren TUDM was helped from the aircraft by Kapten Marzuki and a local villager.

3.    30 July 1992 - One A4-PTM jet belong to No 9 Sqn, Kuantan Air Base  on tactical training flight crash landed on the base runaway due to some technical problem. Both, the instructor pilot, Lt kol Mohd Yusof bin Isa TUDM (43) and the  trainee pilot Lt Zulkifli bin Abdul Rahman TUDM (22) were safe.

4.    28 August 1992 - One A4-PTM jet belong to No 9 Sqn, Kuantan Air Base  on tactical training flight was crashed at Bukit Rangin, Pahang . Both pilots, t

he instructor, Kapt Suri bin Daud () and the  trainee,  Lt Muda Amer Mahmud TUDM
 ejected safely. (Note: I also found from another source that the date of crashed was on the 21st August 1991- need to be verified).


1 JAN - 31 DIS 1993

1.   5 May 1993 - One Alouette 103 helicopter on training flight crashed about 3km from Butterworth Air Base and killed the pilot (instructor) Kapt Shahid bin Yahya TUDM (34) And AQM, F/Sjn Mahadin bin Sham (32).  Berasaskan maklumat saksi (yang memberi maklum balas kepada blog saya), Tragedi ini berlaku di padang Sek Keb Pekan Darat, Butterworth. Pesawat ini mengalami masalah teknikal dan juruterbang ingin membuat pendaratan kecemasan di padang bola sekolah berkenaan. Bagaimanapun melihat ada helikopter mahu mendarat, ramai murid sekolah yang berlari menyerbu ke atas padang (mungkin seronok mahu melihat helikopter). Juruterbang mahu mengelak dari mendarat ke arah kanak-kanak sekolah lalu 'main rotor blade' membidas sebatang pokok kelapa di tepi padang. Helikopter hilang kawalan lalu terhempas.

Kemalangan ini patut memberi pengajaran kepada orang awam khususnya kanak-kanak tentang bahaya mendekati helikopter yang sedang mendarat.

2.   9 July 1993 - One A4-PTM jet from No 6 Sqn crashed near Kuantan Air Base.  The pilot  Kapt Khairani bin Mohamad TUDM ejected safely.

4.   13 October 1993 - Cessna 402B crashed near KL Base airport  and killed both the pilot, Kapt Roslan bin Mansor TUDM and his co-pilot Lt M Fadzil bin Abd Manap TUDM. (the date need to be verified, either 3rd or 13th).

5.   17 October 1993 - One training PC7 aircraft on test flight, faced a landing gear problem, made an emergency landing at Alor Setar base runaway.  Both pilot,  Kapt Rohaizad TUDM and Kapt Kamarudin TUDM were safe unhurt.

6.    19 October 1993 - one A-4PTM Skyhawk crashed landed (or the pilot ejected - need to be verified)  in Kuantan air base after the canopy blown off in the midair. The instructor pilot, Mej Suri bin Daud faced minor injuries and the trainee,  Kapt Sahar bin Ayub TUDM  was unhurt.

7.   11 November 1993 - One PC7 tarining aircraft crash landed at the base due to engine trouble.  The trainee pilot, Lt Mostafa Mahmood Siddiq from Bangladesh Air Force was safe uninjured.

8.    31 December 1993 - NSL332L Super Puma (M36-01) departed from Butterworth to Kangar to pick-up DPM delegation crashed at Kampong Telok, Sg Petani, Kedah. The aircraft captain, Mej Kamarul Bahrain bin Ahmad TUDM, co-pilot Kapt Mohd Fitri @ Chew Ban Chai bin Abdullah TUDM, AQM Sjn (U) Nordin bin Ahmad and stewardess Sjn U (W) Azni binti Alias were killed.  Allahyarham Fitri was my senior in No 2 FTC RMAF Kluang in 1981. 


Nota: secara kebetulan kepada tragedi sedih ini ialah - ditaddirkan Allah, ayah kepada allahyarhamah Sjn U (W) Azni, allahyarham Lans Kopral Alias bin Ibrahim juga terbunuh dalam tratgedi  kemalangan pesawat Caribou TUDM di Telok Cowie, Tawau pada 30 Mac 1970.

1 JAN - 31 DIS 1994

1.   15 April 1994 - One Cessna 402B aircraft from KL Base on training flight was overshot at the runway of Batu Berendam airport, Melaka.  Both pilot were safe.  The student pilot was Lt kol Sreng Sarun from Cambodian Air Force. The instructor (RMAF officer) was......(to be searched).  Later, the investigation found that the incidents was due to the mistake done by the pilot (statement by the then Chief of Air Force).

2.   31 July 1994 - A Caribou on conversion training flight crashed on the attempt for emergency landing at the seaside near Labuan Air Base- The pilot Kapt Raja Mohar bin Raja Bahari TUDM, and two pilots Lt Ali Ridha bin Mohd Yusof TUDM and Lt Mohd Azwan bin Pohari TUDM survived. The other 2 pilots and AQM, Kapt Ahmad Najhan bin Mohamad TUDM, Lt Rosman bin Ramli TUDM and Sjn (U) Ishak Tugiah were killed.

3.  21 September 1994 - One Bulldog 100 aircraft on training schedule crashed in the training area near Kg Kerkau, Mukim Guar Kepayang, Pendang, Kedah about 32km from Alor Setar and killed both the instructor pilot, Mej Kamarudin Ibrahim TUDM (36) and his student Lt M Man Effendy bin Abdul Rahman (Army Aviation Unit). Note: The late Kamarudin is my batch from  Intake TJP37.

13 October 1994

One F5E (29-07) aircraft from No 12 Sqn, Butterworth crashed about 76km southwest of Butterworth after having accident with another fighter aircraft (F-18 belong to Royal Australian Air Force) while performing a joint exercise in Butterworth Base.  The F5E pilot Lt Meor Badrishah bin Mohammad TUDM ejected safely.

One F5E jet aircraft (M29-14) crashed at Tanjung Piandang, Perak (18 nautical mile southwest of RMAf Butterworth base) killed the pilot Mejar Lai Yit Fuen TUDM, 33 years from Changkat, Perak. He took off from Butterworth Air Base for interception and night flying training.The body of the pilot was only recovered on 4th June. Note: I met the late Lai at No.3 FTC when he did his MB339A conversion training in 1985.

31 July 1995

On 7/31/95 M21-13 (c/n # 281) callsign Angkasa 197, with six people on board, took off for a conversion sortie for the newly qualified pilots. The training was carried out in the vicinity of Labuan airfield and part of the circuit had to be flown over water. The pilot planned for a locator approach. Half way through the base turn, the aircraft commander noticed violent fluctuation of the RPM and MAP. It was immediately diagnosed as a "runaway propeller" and the port engine shut down. PAN call was declared.

Immediately after that, starboard engine Zone 2 and 3 fire warning lights illuminated and flames were seen in front of the starboard engine.  The starboard engine was also shut down.  Mayday call was transmitted and aircraft prepared for ditching.  The aircraft impacted the water tail first. The crew in the cockpit immediately abandoned ship through the top escape hatch. One of the aircrew surfaced from the port side of the aircraft. The aircraft finally sank bringing along with it three of the crew. (Capt.Kalam bin Pie (RMAF)

13 October 1994

One Hawk Mk 108 aircraft from No 6 Sqn, Kuantan Air Base  skidded  at Kuching international Airport due to tyre explotion.  The pilot Lt Izwan Zainal Amran TUDM  was safe unhurt. (need more information).


1 JAN - 31 DIS 1996

15 April 1996

MB339A ( M34-08) from No 15 Sqn, Butterworth Air Base on deployment to Kuantan Air Base.  Live rocketry onto a small rocky outcrop in the South China Sea. The aircraft suffered serious airframe failure causing it to spiral into an unrecoverable dive at 2000 ft. Both pilots   Lt Sebastian A William TUDM and  Lt Saiful Azuar Ariffin TUDM ejected and survived and were picked up by passing fishing vessels before being transfered to a Sikorsky S61A4 SAR helo back to Kuantan Air Base.

2.   17 April 1996 - F5E (M29-22) crashed at Butterworth (need to be verified)

3.  18 June 1996 - One Hawk Mk 208 jet from No 6 Sqn, Kuantan Air Base (M40-03) crashed at Labuan AB and both pilots Kapt Chandran TUDM and Kapt Zakaria TUDM ejected safely.

4.   23 July 1996- One RMAF Hawk Mk 208 jet (M40-10) crashed during take off at Labuan runaway - the pilot Kapt Harun TUDM and the copilot ejected safely.

5.   31 August 1996 - S61A Nuri - emergency landing - no casualty.

6.   16 Oct 1996 - One S61A Nuri helicopter on rescue mission was crashed into the sea at Muka Head, Penang, the copilot, Lt Zaili bin Othman TUDM missing in the sea (not found) but the pilot Lt Daud bin Sulaiman TUDM and one RMN AQM trainee,  Bentara Kanan Mohd Hakimi Mansur were survived.  The other crews who also missing were Flt Sjn Tuan Yaakob Yahya and Laskar Udara Kanan Saidon Shaari.

18 March 1997

2 x S61A (23-27 and 23-30) crashed within short period at Gunung Lumaku, Sipitang. 11 killed including; 

From Heli 1 (M23-27)

(1)   Lt Emile Effendy bin Sehu Ismail TUDM (pilot); 
(2)   Kapt Zulkifli bin Othman TUDM (copilot); and 
Sjn (U) Hamdan bin Jais (AQM).

From  Heli 2 (M23-30): 

(1)   Kapt Khairul Anuar bin Mohd Shamsudin TUDM (pilot):
(2)   Lt Zamani bin Mohamed TUDM (co-pilot); and 
(3)   Sjn (U) Abang Wahi A.Saili (AQM).

KUALA LUMPUR 19 Mac - Tentera Udara Diraja Malaysia (TUDM) mencatatkan tragedi nahas pesawat paling buruk dalam sejarahnya apabila dua helikopter Nuri terhempas di kawasan pergunungan daerah Sipitang selatan Sabah, semalam - meragut empat nyawa dan dikhuatiri mengorbankan tujuh lagi.

Inilah kejadian pertama di negara ini yang melibatkan dua pesawat terhempas serentak. Nahas itu berlaku dalam kabus tebal. Bagaimanapun sehingga lewat malam ini, punca kejadian masih belum diketahui dan pihak TUDM enggan mengulas terhadap sebarang andaian termasuk kemungkinan kedua-dua pesawat itu berlanggar atau bergesel di udara.

Helikopter yang pertama membawa empat anak kapal manakala helikopter kedua membawa tujuh anak kapal. Kedua-dua pesawat itu yang berpangkalan di TUDM Labuan terhempas di Bukit Batangga, di Mandulong berdekatan Sipitang sejurus selepas menyelesaikan misi menghantar bekalan untuk pasukan keselamatan di kawasan pedalaman. Pesawat pertama iaitu A 332 berlepas dari Long Pasia manakala pesawat kedua A 334R berlepas dari Maligan.

Mereka yang terkorban dalam pesawat A 332 ialah Leftenan Emile Effendy Sehu Ismail, Kapten Zulkifli Othman, Sarjan Udara Hamdan Jais dan Kopral Udara Wassidin @ Hamirin Inken @ Yingon. Leftenan Emile adalah juruterbang pesawat itu dan beliau dibantu oleh Kapten Zulkifli.

Dalam pesawat A 334R pula, mereka yang dikhuatiri terkorban ialah Juruterbangnya, Kapten Khairil Anuar Mohd Shamsudin, pembantunya, Leftenan Zamani Mohamad, Sarjan Udara Abang Wahi Abang Saili, Flight Sarjan Mohd Amir Klin Abdullah, Laskar (U) Kanan, Mohd Tajudin Chemey.

Seorang jurucakap TUDM ketika ditanya para pemberita memberitahu, "adalah terlalu awal untuk memperkatakan tentang punca kejadian.''

Bagaimanapun Utusan difahamkan, punca kejadian itu menimbulkan `tanda tanya besar' di kalangan pegawai-pegawai kanan TUDM termasuk Panglimanya, Datuk Ahmad Saruji Che Rose. Tiada sidang akhbar oleh Panglima TUDM sebagaimana amalan biasa apabila berlaku sesuatu nahas.

Menteri di Jabatan Perdana Menteri, Datuk Abang Abu Bakar selaku Pemangku Menteri Pertahanan telah memaklumkan nahas tersebut kepada kepada Perdana Menteri, Datuk Seri Dr. Mahathir Mohamad, petang ini. Sehingga petang ini, empat mayat sudah dijumpai. Bagaimanapun pasukan pencari dan menyelamat bersama lima pesawat mendapati ``tiada tanda-tanda ada mangsa terselamat.''

Gerakan mencari akan disambung pagi esok.  Sementara itu, laporan dari Sabah menyatakan operasi mencari dan menyelamat amat sukar kerana kawasan kejadian berbukit-bukau, dilitupi hutan tebal dan jauh terpencil. Operasi tersebut diketuai oleh Panglima Divisyen 4 Udara, Brigedier Jeneral, Goh Seng Toh.

Menurut Kementerian Pertahanan dan polis, pesawat pertama dikesan oleh pasukan pengawal sempadan pada pukul 5 pagi sementara pesawat kedua dijumpai pada pukul 3.45 petang. Kedua-duanya dijumpai di kawasan kira-kira 16 kilometer ke utara kawasan Maligan dekat Gunung Lumaku.

Jarak pesawat pertama dijumpai 1,500 meter daripada pesawat kedua. Kedua-dua pesawat itu tidak kembali ke pangkalannya di PU Labuan pada waktu yang ditetapkan dan telah berlepas dari Long Pasia pada pukul 4.16 petang dan dijangka tiba di TUDM Labuan pada pukul 4.55 petang.

Satu operasi mencari pesawat berkenaan telah dilancarkan pada pukul 6.59 petang semalam dan disambung pagi ini.  Menurut Ketua Staf Operasi Markas TUDM, Mejar Jeneral Datuk Abdul Malek Shahar Harun, Brig Jen Goh telah mengarahkan operasi mencari apabila pesawat berkenaan tidak tiba pada waktu yang ditetapkan.

Sebuah pesawat C 130 yang membuat penerbangan mencari malam tadi telah mengesan isyarat kecemasan di kawasan Malingan.  TUDM juga telah mengerahkan lima buah pesawat - tiga Nuri, sebuah C-130 dan sebuah Beechcraft - untuk operasi mencari dan menyelamat.

Ketua Siasatan Jenayah Sabah SAC II Datuk Mazlan Tyan berkata pasukan mencari dan menyelamat darat, terdiri daripada 30 anggota polis dan pengawal sempadan terpaksa meredah hutan tebal dan mendaki gunung sebelum menuruni cerun kira-kira 156 meter untuk sampai ke tempat kejadian.

1.   4 September 1998 - one MiG29 (M43-17) aircraft crashed at Jaya Gading, Kuantan- the pilot Mej Mohd Fauzi bin Salleh TUDM ejected safely.

2.   10 September 1998 - 40-36 -one Hawk aircraft crashed at Kuala Betis Gua Musang, Kelantan during A to G firing manouvres and the pilot Kapt Jaidev Biaspal a/l Basdaf Biaspal killed.

1.   23 March 1999 - a Caribou (M21-05) aircraft on training flight was crashed at the Kuching AB runaway, and killed all the 5 crews. Three pilots, Kapt Salleh bin Malek TUDM, Kapt Mohd Johari bin Pomin and Kapt Asri bin Abdullah TUDM and 2 AQMs Sjn (U) Mili bin Mat and Sjn (U) Adnan bin Ariffin were killed.  The crash was due to loss of power in one engine during regular training flight on approach into a swampy area at the end of  the runway.

1.   2 October 2000 - One Hawk Mk 108 aircraft (40-06)  crashed landing at Labuan AB (skidded on the runaway) - pilot Lt Abd Majid bin Kassim TUDM ejected safely

LABUAN: An RMAF Hawk Mk 108 fighter jet skidded and crashed while landing at the airport runway here yesterday. The pilot of the two-seater aircraft, Lt Abdul Majid Kassim, managed to eject safely in the incident which happened at 1.38pm. Lt Abdul Majid underwent a check-up at the Armed Forces Medical Centre in Labuan.

The strong wind causing the pilot to have difficulty in controlling the aircraft. The aircraft came to a stop on its own by the side of the runway. The aft from the No. 9 Squadron of the Kuantan Air Base was assigned to Labuan to help in surveillance operations in Sabah.
The RMAF has set up a board of inquiry headed by Mejar Mohamad Nor Rashid Samion with Mejar Azahari Mohamad Yusof and Mejar Atan Bakhti Abdul Malek to help investigate the incident.

    1. I recovered the remains of my good friend, the late Lt Faizal from that 16 feet hole in the paddy field in Kampung Tokai, Pendang. I was with his remains throughout until he was flown away in the C130 from KTU, Alor Setar.

    Kronologi : Semasa cuba mendarat kecemasan di padang sekolah akibat dari blade gear failure, pelajar sekolah menyerbu ke padang menyebabkan pilot mengelak dan bilah rotor membidas pokok kelapa dibahagian hadapan cockpit akibat heli hilang imbangan.
    Pada pendapat saya pihak awam perlukan kesedaran tentang bahaya mendekati heli yang sedang mendarat kerana boleh menyebabkan kecederaan atau kematian.

  9. Waalaikumsalam. Maklumat yg saya tahu mendiang mejar gabriel bija joel terbunuh selepas helikopter awam jenis bell 205 jet ranger terhempas di kawasan paya Bario sarawak. Tahun kejadian saya tak ingat. Mendiang sewaktu itu telah bersara dari TUDM. Saya pernah tengok gambar mendiang dan penumpangnya serta helikopternya hangus dalam hutan paya. Takziah

    Pesawat nuri membawa penumpang terdiri dari anggota tentera dan 3 orang awam dari jabatan ukur dan pemetaan serta membawa bahan binaan untuk batu sempadan (Ops Ukur Sempadan) terhempas di Batu melintang kelantan. Hanya seorang penumpang jabatan ukur yang terbunuh.

  17. Saya pernah terbang bersama arwah Lt Fitri Chew circa 86/87. Kami dalam penerbangan ujian atas nuri pergi balik dari KL-Kuantan-Kuching-Bintulu-Labuan-Layang2-Labuan-Tawau. Beliau seorang yang peramah dan baik. Bersama yang terkorban seorang Flight Sargeant Sabri. Semoga roh mereka di cucuri rahmat Allah.
